Looking for a great opportunity?  We seek a Registered Nurse with previous clinical nursing experience for the role of RN – Transitional Care Unit.  In this role you will have the ability to make a huge positive impact on the communities as they help patients transition to home so they may live independently or with some assistance. The patients in this unit are often return patients, so this department does get to know the patients and their families very well.  You will be responsible for providing and coordinating comprehensive care.  You will provide care to all age groups through the proper utilization of the nursing process; assessment, formulation of nursing diagnosis, planning, implementation and evaluation of plan of care in accordance with established standards, policies, procedures, protocols and guidelines.  You will be responsible for supervising nursing staff to care for groups of patients.  Some additional tasks are making referrals to outpatient services, identify community resources that can help, assess and facilitate discharge needs and provide extensive patient and family education. 

Candidates must be a graduate of an accredited school of nursing and be licensed as a Registered Nurse in the state of Illinois (or have the ability to gain this licensure).  Our ideal candidate will have previous acute care nursing experience.  BSN is also a plus.  New grads are welcome to apply for this role. Must have advanced skills in reading, writing ,and oral communication. Must demonstrate skills in the use of clinical information systems, basic skills in keyboarding and use of computer systems.
