We’re working on behalf of a healthcare organization that features a 90-bed acute care, community hospital, a medical group practice with nearly 20 clinics, a long term care center, and an ambulatory surgery center.  The long term care facility provides both long term care and short term rehabilitation. Their team of dedicated care professionals has a passion for long term care and is focused on the individual needs and unique personalities of each resident. The care team consists of registered nurses, licensed practical nurses and certified nursing assistants, working under the supervision of the Director of Nursing. Social services, rehabilitation and therapy staff, activity coordinators and volunteers work hard to make it truly feel like home for all residents.

Looking for a great opportunity?  We seek a Registered Nurse with licensure in the state of Wyoming (or a compact license) and previous MDS experience for the role of MDS Coordinator.  In this role you will work under the supervision of the Administrative Director of Nursing and Director of Nursing to assure that the federally mandated MDS process is completed accurately and timely for the State of Wyoming. You will play a critical role in the contribution to the total plan of care for each resident and ensure compliance with state and federal regulations. You will oversee complete MDS data from other disciplines to ensure accuracy of the MDS. You will implement and collaborate with other departments to ensure excellence in resident/patient and family care. You will ensure the integration of departmental goals for improvement across the care continuum.

Candidates must be a graduate of an accredited school of nursing and be licensed as a Registered Nurse in the state of Wyoming (or have compact state license).  Qualified applicants will preferably possess one (1) or more years of experience in an MDS Coordinator role or have relevant experience.  Must have knowledge of regulatory compliance and skilled nursing operations. Must be able to interface and maintain effective relationships with all departments and employees in a team-oriented environment.
