Looking for a change of scenery?  We seek candidates with three (3) or more years of experience for the role of Outpatient Physical Therapist.  In this role you will join a 25-bed hospital that has been recognized by Centers for Medicare & Medicaid Services (CMS) as one of the modern hospitals which are scientifically measured and assessed to have high-quality health care services for promoting health and improving the quality of life.perform comprehensive, caring evaluations on new patients, establish achievable goals, and develop actionable treatment plans.  You will provide on-going physical therapy services that effectively and efficiently comply with treatment plans. You will take ownership of obtaining necessary additional information and suggesting revisions in treatment as appropriate. You will document all evaluations, treatments and other pertinent patient interactions.

Candidates must have a Bachelors Degree Masters Degree or Doctorate Degree in Physical Therapy from an accredited institution. Must be licensed or eligible for licensure in the state of Indiana as a physical therapist. If eligible for licensure must obtain license within three (3) months of employment. Must be certified in CPR by American Heart or American Red Cross or must become certified within 90 days of employment. Must maintain certification according to expiration date. American Physical Therapy Association membership preferred.
